Signature Sheet

The Signature Sheet contains signatures from all parties needed to approve a project, including the Office of the Vice President for Information Technology, Team Coordinators, and Customers' Signatures. In addition, the signature sheet includes a brief description of the project including a start and end date and a list of project team members. Each page of the SLA including Scope of Work, Timelines, Budget, and special Terms and Conditions, contain initial sections at the bottom so every page will be initialed by approving parties.

Scope of Work

The Scope of Work contains two sections - a description of the start up project and a description of the maintenance. The start up portion outlines what and how each party will contribute to the project. What will IT do? What will the customer do? What does each expect from the other? It will include measures and reports which will be used to evaluate the progress of each project. The maintenance portion describes the maintenance stage of the completed project and again states the expectations for IT and the customer.

Timelines

The Timeline section of the SLA will outline the steps and deadlines for the duration of the project. Milestones are identified and include a description of the progress required, the expected completion date and a sign off. A maintenance begin date is identified as well.

Budget

The Budget section of the SLA specifies project and maintenance funding including what Information Technology will contribute and what outside customers will contribute, cost breakdowns and payment schedules for both the project itself and the maintenance phase.

Special Terms and conditions for the project are listed here. Much of this section is filled out as the work becomes better understood.
