In consideration for the use of Information Technology telephone service, I agree to the following:

All charges incurred are my responsibility. The balance due for my monthly service is due upon receipt. If it becomes pas due, I understand that my service may be terminated. I will contact the Help Desk at 797-HELP (797-4357) immediately if my account is in arrears. I understand that Utah State University may place a hold on my registration and transcripts and if necessary refer my account to collections until my account is current and that the information may be reported to the Credit Bureau. I will direct all questions regarding billing promptly to Information Technology.
I understand that I must contact the Information Technology office when I move and give notice of terminating my service. I am responsible for all charges incurred until my service has been removed from the system.
